November’s Health and Care Integration bulletin now available

The November edition of the Leicestershire Health and Care Integration bulletin highlights joint working on delayed transfers of care, the completion of Phase two of the SIMTEGR8 project and an update from the Falls programme.

There is also a First Contact Plus case study demonstrating the value of the Warm Homes service, news from the Shepshed walking for health group who now have over 100 registered members and the launch of the Self-care campaign this Autumn.

The September edition of the Leicestershire Health and Care Integration bulletin features updates on the Integrated Discharge Team’s work to develop a patient tracker and the allocation of the final three Help to Live at Home (HTLAH) contracts to new providers.

August Health and Care Integration bulletin now available

In the August edition of the Leicestershire Health and Care Integration bulletin we feature an update from the Home First workstream of the STP on the Integrated Discharge Team that has recently started working on some of the busiest wards at the Leicester Royal Infirmary.

There are also case studies from First Contact Plus and the Hospital Housing Enablement Team that demonstrate the wide variety of advice and support these services can offer, as well as links to our new videos on the eFrat app and the Hospital Housing Enablement Team.

Health and Care Integration bulletin

The June edition of the Leicestershire Health and Care Integration bulletin is now available.

This edition includes an update on the work we are doing to prevent falls including a new video featuring the electronic Falls Risk Assessment Tool (eFRAT) app and the Unified Prevention Board’s recently agreed outcomes.

We also have an update on how plans to integrate adult health and social care Points of Access across Leicester, Leicestershire and Rutland are progressing.

Support for Carers

The efforts being made by Leicestershire County Council to reach out to carers and their families have been acknowledged by NHS England.

The carers’ health and wellbeing service has been highlighted in a new NHS report as a good example of an organisation working closely with GPs to provide backing for the often-unsung heroes.

The service is backed by the county council, along with health partners, and one of the key components involves well-being workers from Voluntary Action South Leicestershire (VASL) linking up with the GPs to offer advice and support to carers.
